This Memorandum of Agreement (“MOA” or “Agreement”) constitutes the entire agreement between the State of Alaska, Department of Health and Social Services,1 Division of Public Health, Section of Health Planning & Systems Development, SHARP Program (hereinafter collectively “SHARP”), the above-named healthcare professional who participates in the SHARP Program (hereafter, “Practitioner” or “Clinician”), and an eligible health care delivery Site (hereafter, “Site”). These entities may hereinafter be referred to collectively as the “Parties.”
