This Agreement for Independent Contractor, Personal Services, (“Agreement”) is between BAYLOR UNIVERSITY, a private, not-for-profit university, with its principal place of residence in Waco, Texas (“Baylor”), and (Contractor’s Full Name), an individual residing in Waco, Texas (“Contractor”), and shall be effective as of (date) (“Effective Date”), regardless of the date this Agreement is signed. (Baylor and Contractor are collectively, “Parties”). For good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the Parties enter into this Agreement in accordance with the following terms and conditions.
