THIS AGREEMENT is effective July 1, 2018, by and between the City of Gallup, a Governmental entity, referred to as "City" and having an address of 110 West Aztec Ave., Gallup, New Mexico 87301, and to Rehoboth-McKinley Christian Health Care Service, Inc., a non-profit corporation, herein called referred to as "Contractor" whose address is 1901 Red Rock Dr., Gallup, NM 87301. The purpose of this Agreement is to provide for the funding with Behavioral Health Investment Zone Grant Funds by "City" to "Contractor" in providing work services, case management, aftercare and GED Services.
