The member has provided payment for membership plan _______ which provides Membership hours of __________ in the sum of $ ___________ which covers a period of ________ . In consideration of the payment for the Membership as described, American Home Care Services, (heretofore known as AHCS) agrees to provide payment for the home care services provided as indicated within the plan terms and conditions of the member’s selected plan. Further explained in Section One below according to and subject to the limitations, terms, and conditions of this contract (hereinafter Agreement). The Member will be referred to in this Agreement as the Member. AHCS Home Care Services, Inc. will hereafter be referred to as “AHCS” in this Agreement.
