A separate copy of this waiver must be completed for each youth traveling to the School-Based Health Alliance 2016 National Convention in Arlington, VA (the “Convention”). Each youth must submit a copy of this form signed by the youth, parent/guardian (if under 18 years of age), and attending chaperone to attend the 2016 National Convention. By signing this waiver, you agree that you are giving up legal rights and remedies available to yourself and your family. Read and complete this form carefully.
