This Before/After School Program Services Contract (“Contract”) dated «ContractDate» (“Effective Date”), is made by and between «School District Name» School District (“District”) and «Company Name» (“Contractor”). This Contract shall govern all provisions for services which are to be delivered by Contractor to District. This Contract is based upon the following Recitals and subject to the Terms and Conditions mutually agreed upon by the parties, and each of them.
