THIS IS AN AGREEMENT FOR MEDIATION/ARBITRATION IN ACCORDANCE WITHMediation/Arbitration Agreement • November 27th, 2020
The parties appoint Michelle Hayes, B.S.W., M.S.W., AccFM (hereinafter referred to as the “Mediator/Arbitrator” or as the “Mediator” or as the “Arbitrator”) to mediate/arbitrate the custody/parenting issues, as the sole arbitrator pursuant to the Arbitration Act, S.O. 1991, c.17 as amended by the Family Statute law Amendment Act. This document shall constitute a submission to arbitration pursuant to the provisions of the Arbitration Act as amended by the Family Statute Law Amendment Act.
