This Membership Services Agreement (“Agreement”) is entered between Jacobson Medical, PLLC, a Nevada limited liability company doing business as Wishing Wellness Medicine Wishing (“Company”) located at 10120 South Eastern Avenue #100, Henderson, NV 89052 or 10161 Park Run Dr. Suite #150, Las Vegas, NV 89145 (“Clinic”) and the patient (s) on the signature page attached to this Agreement (“Patient”). If the signature page of this Agreement indicates that the Patient is a couple or a family, then any reference in this Agreement to Patient refers to each person.
