This Administrative Services Agreement ("Agreement") between United Behavioral Health (“Optum”), a California corporation, and its wholly owned subsidiary, U.S. Behavioral Health Plan, California (“USBHPC”) and Lodi Unified School District (“Customer”) is effective July 1, 2021 (“Effective Date”). This Agreement covers the services Optum is providing to Customer, either directly or in conjunction with one of Optum’s affiliates.
