This ESIGN Disclosure and Consent (the “agreement”) applies to information and disclosures related to a health savings deposit account (an “HSA") that you open with UMB Bank, n.a. (the "Bank", "we", "us", and "our"). This disclosure provides you with important information that you should consider before giving your consent.
