(“JWCH”), a California nonprofit corporation located at 1910 W Sunset Blvd, Suite #650, Los Angeles, CA, and the County of Los Angeles (“County”) herein referred to as “Agency”, which includes the following County Departments as “Participating Agencies” to this agreement: Chief Executive Office (“CEO”), Department of Health Services (“DHS”), Department of Mental Health (“DMH”), and Department of Public Health (“DPH”).
