The Health and Human Services Commission (HHSC) and South Plains Community Action Association, Inc. (Grantee), and collectively the “Parties” to that certain Primary Health Care (PHC) Services agreement denominated as HHSC Contract No. HHS000697900023 (the “Contract”), now want to amend the Contract.
