This purchase agreement (this “Agreement”) is entered into by and between [Legal Name of SBT Supplier] (“Supplier”) and CVS Pharmacy, Inc., a Rhode Island corporation, on its own behalf and on behalf of its relevant store subsidiaries and affiliates (“Retailer”), and will be effective until terminated by either party upon thirty (xx) days’ written notice to the other party. [Legal name of SBT processer] is the “Designated SBT Processer” for this Agreement.
