THIS AGREEMENT, made this day of , , between the SOCIETY FOR DESIGN ADMINISTRATION (SDA), a non-profit corporation in the State of Washington, whose principal office is at 8190-A Beechmont Avenue #276; Cincinnati, OH 45255-6117 (hereinafter referred to as “SDA”), and the CHAPTER of SDA, a non-profit (if applicable) association (corporation) in the State of (hereinafter referred to as “CHAPTER”), collectively referred to as the "Parties" and severally as the "Party".
