GUEST PERMISSION FORMNovember 18th, 2021
Students who bring guests will be required to fill out this contract before receiving approval to bring a guest. Students are only allowed one guest. If approved by administration, this contract will be forwarded to the advisor in charge of the dance. E-K Central School staff will not be responsible for faxing these forms to area schools. Additionally, the guest must be accompanied to the dance by their Edwards-Knox host. Guest’s behavior will be the responsibility of the host student.
