I, , an employee of and a sworn law enforcement officer, do hereby willfully and voluntarily consent to participate in the Police Officer’s Assistance Trust (POAT) 5K Run Fundraiser while wearing my Police Department Class B uniform and appropriate running shoes. Although in uniform, I fully understand and agree that I will be participating in this 5K Run Fundraiser in an off-duty capacity, and will not be performing any law enforcement function. I further understand and agree that I will not be compensated in any way for my participation in this fundraiser and any injury or harm incurred will not be considered in the line of duty for consideration as a worker’s compensation claim.
