This is a statement in which you are informed of the established practices for the successful completion of the Tarpoon Lagoon Divers Academy (hereby referred to as ‘Tarpoon’) Diver Training Programs. These practices have been compiled for your review and acknowledgment and are intended to increase your comfort and safety in diving. Your signature and initials on this statement is required as proof that you are aware of the requirements of the Diver certifications. Read and discuss the statement prior to signing it. If you are a minor, a parent or guardian must sign as well.
