Participation Agreements with Providers: Pacific Private Practice Network, Inc., (P3N) has entered into participation agreements with each of the P3N participating physicians listed in P3N’s Provider Directory pursuant to which the providers agree to participate in the ACO and are eligible to receive a portion of any savings generated. These agreements set forth the duties and responsibilities of Pacific Private Practice Network, Inc., and the participating physicians to deliver high-quality, coordinated care and improve the overall health of our Medicare beneficiaries.
