The Department of State Health Services, Health Service Region 8 (“System Agency”), a pass- through entity, and Wilson County ("Grantee"), who are collectively referred to in this Amendment as the “Parties” or singularly as the “Party,” to that certain Cities Readiness Initiative grant Contract, effective July 1, 2018, and denominated System Agency Contract No. HHS00145900001 (the “Contract”), as amended, now desire to further amend the Contract.
