This Medical Informatics Data Sharing Agreement (the “Agreement”) effective as of the ___ day of Month, 20XX (the “Effective Date”) is entered into by and among the Unnamed Hospital (“UHOSPITAL”), a not-for-profit corporation, the University of UnnamedState (“University”), an agency of the State of UnnamedState, on behalf of the University of UnnamedState Medical Center and its UnnamedState City School of Medicine and Schools of Nursing and Allied Health (together, “UUMC”), and Unnamed Physicians Practice, a not-for-profit corporation (“UPP”).
