All black belt testing is administered by World Kuk Sool Association (WKSA), Inc. In order to begin your Black Belt testing, you must complete this form and pay the testing fee of $400.00 (US dollars) payable to World Kuk Sool Association, Inc. PRIOR to the 1st Testing day. By entering into this Agreement, the Candidate and/or the parent/guardian understand and agree to this Agreement, and also understand and agree that the fee is 100% NON- REFUNDABLE and NON TRANSFERABLE and shall NOT be refunded for any amount under any circumstances.
