Assumption of Risk: I HEREBY ACKNOWLEDGE AND AGREE that I wish to participate in the name of event (“event”). I understand that the event includes potentially dangerous activities such as, but not limited to, the following: list of activities (i.e., running, swimming, making contact participants, etc…).
