This contract has been designed to bring understanding amongst student athletes, their parents/guardians and school personnel. We ask that all participants and parents read and sign this contract, indicating full knowledge of the rules, regulations and commitment that participation in sports programs at Hillsboro-Deering Middle School demands, as well as reading the Concussion Fact Sheets for athletes and parents, and the Return to Learn Form regarding head injury or concussion information. Please note: Return to Learn before Return to Play β Any student-athlete that suffers an injury requiring accommodations to access their normal academic programming shall not return to athletics until accommodations are no longer required.
