CARE SERVICES (this “Agreement” or “Third Amended and Restated Agreement”), is made this day of , 201 (the “Effective Date”) by and between INDIAN RIVER COUNTY HOSPITAL DISTRICT, an independent special taxing district of the State of Florida, hereinafter referred to as DISTRICT, and INDIAN RIVER MEMORIAL HOSPITAL, INC., a Florida not-for-profit corporation, hereinafter referred to as MEDICAL CENTER (collectively, “Parties”).
