By signing this form, I authorize DePauw University to charge my student account the following fee for the accompanying package indicated below and an additional fee for an extra event, if indicated. I understand that the amount of accompanying time in the package I select below includes both the time I work with the staff accompanist this semester during lessons and rehearsals and at my end-of-semester jury performance. Furthermore, I understand that this package fee does not cover extra rehearsals for or participation in events such as a junior or senior recital.
