This Wellness Program Services Agreement (this “Agreement”) is effective the 1st day of October 2024 (the “Effective Date”) by and between TMA @ Your Service, LLC (doing business as “Wellworks For You”) (“Wellworks”), having a principal place of business located at 70 East Lancaster Avenue, Frazer, PA 19355, and the party identified (or parties collectively identified) as “Client” on the signature page of this Agreement (“Client”). Each of Wellworks and Client may be referred to as a “Party” and together they may be referred to as the “Parties.”
