In consideration of the opportunity to participate in soccer, soccer-related and other sports-related activities (collectively, the “Activities”), and for the Activities in which I actually participate, at the any fields or facilities at which Summer “I” League may be hosting Activities (collectively, the “Facility”), I, for myself and my personal representatives, assigns, heirs and next of kin:
