This Program Academic Affiliation Agreement (“Program Agreement”) is entered into by and between The University of Texas Health Science Center at Houston (“Institution” or “University”), on behalf of its McGovern Medical School (“Department”), and the Department of State Health Services (“DSHS”, “Host Agency” or “Agency”), on behalf of any and all of its participating organizational components and the facilities that house them (“Facilities” or “Educational Sites”). The Institution named in this Program Agreement, and any and all participating DSHS programs are collectively referred to herein as the “Parties."
