This Patient Service Agreement is entered into between Upstate Concierge Medicine, PLLC (“UCM”) DBA, United Concierge Medicine, and the undersigned patient (“Patient”) for membership in United VIP Medicine and access to the services defined herein (the “Agreement”). UCM and Patient shall hereinafter be referred to as individually the “Party” and collectively as the “Parties.”
