This Service Agreement (this “Agreement”), which includes the appendices and statement of work attached hereto, is between Bend Financial, Inc., a Delaware corporation with its primary place of business at 115 Broad St., Boston, MA, 02110 (“Bend”) and the company accepting this agreement online (“Employer”), and is effective as of the date accepted by the Employer (the “Effective Date”). Each of Bend and Employer may be referred to herein individually as a “Party” or collectively as the “Parties.” This Agreement includes, collectively, (1) this Cover Sheet, (2) the attached Pricing Exhibit, and (3) the attached Standard Terms and Conditions (including Exhibit A). The Employer has caused this Agreement to be executed by a duly authorized representative.
