This Amendment No. 1 (the “Amendment”) is made by and between Verizon North Inc., f/k/a GTE North Incorporated, Verizon South Inc., f/k/a GTE South Incorporated (“Verizon”), a Wisconsin corporation with offices at 8001 West Jefferson, Ft. Wayne, Indiana 46804, and American Fiber Network, Inc., a corporation with offices at 9401 Indian Creek Pkwy, St. 140, Overland Park, Kansas 66210 (“AFN”), and shall be deemed effective on October 12, 2004 (the “Amendment Effective Date”). Verizon and AFN are hereinafter referred to collectively as the “Parties” and individually as a "Party". This Amendment covers services in Verizon’s service territory in the State of Illinois (the “State”).
