This Human Material Transfer Agreement (“Agreement”) will be effective on the date of last authorized signature below, (“Effective Date”). The purpose of this Agreement is to transfer NCI-sponsored clinical trial biospecimens and associated data from [Provider Name], hereinafter referred to as “Provider”, to Dana-Farber Cancer Institute, Inc., Leland Stanford Junior University, the University of Texas MD Anderson Cancer Center, and the Icahn School of Medicine at Mount Sinai (each a “CIMAC”); and Dana-Farber Cancer Institute, Inc. (the “CIDC”) who will receive the data related to the biospecimens, (jointly the “Recipients”). Each Recipient and the Provider is hereafter individually referred to as a “Party” and jointly as the “Parties”.
