having its principal office at (“Business Associate”) and The South Dakota State Employee Health Plan administered by The South Dakota Bureau of Human Resources (the “Covered Entity” or the “State”) (each a “Party” and collectively the “Parties”), and is made a part of that certain service agreement(s) (the “Service Agreement”) between the parties pursuant to which Business Associate provides a service or services to Covered Entity that involves the use or disclosure of Covered Entity’s Protected Health Information (“PHI”).
