This User Agreement (the “Agreement”) sets forth the terms and conditions pursuant to which COVID.MD Health, Inc. (“COVID.MD” or “us”, “we”, “our”) will provide you (“you” or “your”) with access to use any of the web-based software, products, or applications made available on our website located at www.fruitstreet.com (including any successor site) (the “Website”), or our mobile application (the “COVID.MD Mobile Application”) (collectively, the “Platform”). You agree to be bound by this Agreement on the date (the “Effective Date”) you click "Sign up" or "I agree", creating your account on the Platform, or otherwise indicating your assent. If you are entering into this Agreement on behalf of an entity, you represent that you have the authority to and you hereby bind such entity to this Agreement.
