This Notice Amendment supplements, amends, and is made a part of the agreement (“Agreement”) presently in force between ValueOptions of California, Inc. (“VOC”) and Participating Provider (“Provider”). This Notice Amendment is issued by VOC in accordance with the provisions of the Agreement which permit VOC to issue notice amendments as necessary to maintain compliance with changes in law, and this Notice Amendment does not require the consent of Provider.
