In this Patient Agreement and Consent (this “Agreement”), the terms “you” and “yours” refer to the person who is creating a patient account with Forward, or in the case of a minor, “you” and “yours” refer to and include (i) the parent or legal guardian who provides consent to the creation of such account by such minor, and (ii) the minor for whom consent is being provided.
