THIS BUSINESS ASSOCIATE AGREEMENT (“Agreement”) is made by, and between __, represented herein by the undersigned authorized individual, having its principal offices located at (“Business Associate”) and The Puerto Rico Health Insurance Administration (PRHIA) (“Covered Entity”), represented herein by Ricardo A. Rivera Cardona, who is authorized to act for and on behalf of Covered Entity and with its principal offices at San Juan, Puerto Rico. Covered Entity and Business Associate, collectively, may hereinafter be referred to as the “Parties.”
