Complete the attached Agreement by filling in the blank lines with name and address of business or building name, legal description and tax ID number of the property, and sign at the bottom of the last page. Please note that the Owner’s signature will need to be attested to. Return the signed Agreement to the Department of Community Development & Growth Management (lower level of the Municipal Center at 9446 Raymond Avenue, Oak Lawn, IL), along with a Certificate of Insurance covering comprehensive liability for both the Owner and the Village as additional named insured (see #6 of Agreement). The Certificate of Insurance will need to be renewed each year.
