Corporation, a Delaware corporation (“Supplier”), 800 Arthur Ave., Elk Grove Village, IL 600007, are this day entering into a contract based on Genfare Sales Quote No. 5030177 and Quote No. 5030180, dated January 6, 2022, and, for their mutual convenience, the parties are using the standard form contracts provided by Supplier, titled “Genfare Subscription and Services Agreement.” This Addendum, duly executed by the parties, is attached to and hereby made a part of Supplier's standard form contracts and together shall govern the use of any and all equipment, software and services purchased, subscribed to and/or licensed by the County whether or not specifically referenced in the order document.
