Current Vaccinations/Veterinarian Information: By signing this contract, owners verify their pets are current on Rabies, Distemper and Parvo-Virus. Proof of Vaccination or current Titer testing shall be provided to Sprockett’s Doggy Day Camp Inc. upon request as well as current Veterinarian information.
