This Amendment to Contract No.: DMS-14/15-018 (the “Contract”) is by and between the State of Florida acting through the Department of Management Services (the “Department”) and Sun Life Assurance Company of Canada (“Service Provider” or “Vendor”), collectively known as the “Parties.”
