In consideration of participating in Accelerated Rehabilitation Centers, Ltd.’s (“Accelerated”) and/or Athletico, Ltd., Athletico Management, LLC or one of their respective affiliates (“Athletico”) anterior cruciate ligament (ACL) screening program (the “Program”). I represent that I understand the nature of this Program and that I am/my child is qualified, in good health, and in proper physical condition to participate in such Program. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue my/my child’s participation in the Program. I fully understand that this Program involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my/my child’s own actions, or inactions, those of others participating in the Program, the conditions in which the event takes place, or the negligence of the “released parties” named below; and that there may be other risks either not known to me/my child or not readi
