This Subscription Agreement to Provide Dental Benefits (“Agreement”) is made and entered into the lst day of December 2014 by and between CITY OF OVERLAND PARK, KANSAS, hereinafter referred to as "Employer", and DELTA DENTAL OF KANSAS, INC., hereinafter referred to as “DDKS”. The initial term of this Agreement shall be from January 1, 2015 through December 31, 2015, inclusive, and shall renew for two subsequent one-year terms, subject to the provisions of Section VII. This Agreement is the controlling document for all benefits, terms and conditions and supersedes all other written or verbal communication regarding the Plan.
