This BUSINESS ASSOCIATE AGREEMENT (this “Agreement”) is entered into by and between you (“Covered Entity”) and Avhana Health, INC (“Business Associate”), and shall be effective the date of signing up for an account and using the services provided by Avhana Health (the “Effective Date”) or such other mandatory compliance date as may be set for the Privacy Rule enacted pursuant the Health Insurance Portability and Accountability Act of 1996, as amended, and its implementing regulations (“HIPAA”), including 45 C.F.R. Parts
