UNDERGRADUATE STUDENTS AT BOONE HEALTH (this “Agreement”) is made and entered into this 12th day of March, 2024 (the “Effective Date”), by and between City of Columbia (“School”) and CH Allied Services, Inc. d/b/a Boone Hospital Center, on behalf of itself and its subsidiaries and affiliates, including Boone Physician Services, LLC and CHAS Physician Services, LLC (collectively, “Facility”). School and Facility are hereinafter referred to individually as a “Party” and collectively as the “Parties.”
