[New subnetwork organization/entity name] and Catholic Family Center (CFC) have come together to provide support to people who need access to health and social determinants of health (SDoH) related services through the 360 Collaborative Network (360CN) in the Finger Lakes, NY area. The 360CN is a collaborative of local organizations working together to provide service coordination to individuals, including service members, community residents, in the Finger Lakes, NY and the surrounding area. [New subnetwork organization/entity name] and CFC may also each be referenced as a “Party” individually or “Parties” collectively. The Parties desire to enter into this Memorandum of Agreement (MOA) setting forth the services, roles, and responsibilities to be provided by the Parties.
