This Local Exchange Telecommunications Services Resale Agreement (“Agreement”), is effective as of the day of 199_ (the “Effective Date”), by and between Cincinnati Bell Telephone Company, an Ohio corporation with offices at 201 E. Fourth Street, Cincinnati, Ohio 45201 (“CBT”), and Frontier Telemanagement Inc., a Wisconsin corporation with offices at 180 South Clinton, Rochester, New York 14646 (“Frontier” ).
