This Agreement, dated for reference purposes as of , is between the County of Suffolk ("County"), a municipal corporation of the State of New York, having its principal office at the County Center, Riverhead, New York 11901, acting through its duly constituted Department of Public Works, 335 Yaphank Avenue, Yaphank, New York 11980-9744 ("Department"); and ("Consultant") having its principal office at . The parties hereto desire to make available to the County of Suffolk, professional services in connection with hereinafter called the “Project”, pursuant to Resolution No. , of the County Legislature. The Commissioner of Public Works (“Commissioner”) recommends that the Consultant be retained for the purpose of rendering consulting services from time to time on various projects as requested by the Commissioner of Public Works.
