THIS PERSONALIZED CARE MEMBERSHIP AGREEMENT (this “Agreement”) is made effective as of , (the “Effective Date”) by and between the undersigned member and, if applicable, additional members listed on Schedule 1 hereto (each, a “Program Member”), and RBB Personal Care, LLC, a Mississippi limited liability company, having an address of 4603 Hospital Rd. Pascagoula, MS 39581 (“Personalized Care Practice”; and together with Program Member(s), the “Parties”).
