This annual contract, made April 1, 2019, between New York University School of Medicine ("NYUSoM"), an administrative unit of New York University, having offices at 550 First Avenue, New York, New York 10016, and <<FirstName>> <<MiddleName>> <<LastName>> <<Credentials>> ( the “House Staff Officer"), is entered into for a term of one (1) year, beginning <<StartDate>> and ending <<EndDate>>. This contract serves as a single statement of understanding between the House Staff Officer and NYUSoM. Contracts will be issued each year that a House Staff Officer participates in a NYUSoM Residency or Fellowship Training Program (“training program”).
